Welcome to Module 1, titled Person-Centered Practices for Home and Community Support Professionals! This is the first module of your HCSP training. In this first module, we will lay out the foundation of all home and community support, which is person-centered practices. We will explore how to provide support that truly centers on the individual, including their goals, values, and right to lead their own life. You will learn what person-centered practices look like in action, how to help shape and carry out individualized plans, and how to build trust by listening closely and adjusting as needs evolve. These skills will help you deliver meaningful, respectful support that empowers the individuals you serve every day.

Foundational Module 2: Introduction to HCSP Role and Ethical Considerations

In this module, we will explore the essential responsibilities of an HCSP, strive to understand individuals' rights, learn professional and ethical standards, establish appropriate boundaries, and cover privacy requirements. Through these lessons, you will build the foundation needed to provide person-centered support and develop trusting relationships with the individuals you serve.

Foundational Module 5: Encouraging Community Living Skills and Supports for Individuals
Welcome to Module 5, titled Encouraging Communing Living Skills and Supports for Individuals, of the HCSP training. In this module, we will explore how to support individuals in building fulfilling and connected lives within their communities. You will learn what “community” truly means, how to assist with daily living and life skills, and how to implement personalized plans. You will also learn how to provide support that empowers individuals to participate fully in their communities how they want to—at home, at work, and beyond.

- Meaning of "community" and your role in fostering belonging
- Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs)
- Implementation and adaptation of person-centered support plans
- Day-to-day support that promotes independence, diginity, and engagement
Foundational Module 6: Importance of Documentation and Reporting
Welcome to Module 6, titled Importance of Documentation and Reporting, of the HCSP training. In this module, we will explore how strong observation and documentation skills help protect, support, and advocate for the individuals you serve. You will learn how to recognize important changes, record key information, and communicate with the right people at the right time. Clear, person-centered documentation plays a vital role in promoting safety, health, and quality care.

Fundamentals Module 5: Safety, Risk Management, and Crisis

Safety is a core part of providing quality support, but it does not mean doing everything for someone or taking away their choices. You will explore how to recognize potential risks, respond to challenges early, and work with others to prevent crises before they happen. You will also learn what to do during a crisis, how to support someone through it, and how to connect them with resources afterward. You will learn how to recognize warning signs, make safety plans, and create environments that reduce harm without taking away their dignity. By the end of this module, you will have the knowledge and tools to support individuals in making informed choices while helping to keep them and those around them safe and well.
